This is one of three optional mid-clerkship evaluation forms (the others are the Peer Assessment Tool and the Case Based Discussion Evaluation form). The purpose of this tool is to allow students to get feedback from another member of the healthcare team. It should not be the preceptor, but may include a nurse, office manager, non-preceptor physician, case manager, etc.

Case Based Discussion Evaluation Form - KCUMB
Contact Person: : Linda Adkison, PhD ladkison@kcumb.edu
The purpose of this tool is for the student to receive feedback about a patient/case the student has done recently. Typically, the preceptor will ask the student questions about a patient and probe the student’s understanding of the presentation and diagnosis. The value of these three optional tools is that they provide documented feedback that students may use to improve performance.
