General Information

The Sam Houston State University College of Osteopathic Medicine is the university’s eighth college and one of only three colleges of osteopathic medicine in Texas. The College is housed in a four-story, 107,000-square-foot facility located in the Grand Central Park development in Conroe, Texas.

For more than 140 years, Sam Houston State University has served the evolving needs of Texas. Mission Statement

The mission of the Sam Houston State University (SHSU) College of Osteopathic Medicine (COM) is to prepare students for the degree of Doctor of Osteopathic Medicine with an emphasis toward primary care and rural practice, to develop culturally aware, diverse and compassionate physicians, who follow osteopathic principles, that are prepared for graduate medical education, and will serve the people of Texas with professionalism and patient-centered care. 

A significant part of the mission of the COM is to increase the physician workforce in the eastern region of Texas and to increase access to primary care. The COM will accomplish this by recruiting qualified applicants from areas to which they would likely want to return and establish their practice.

The state-of-the-art facility is housed in a four-story, 107,000-square-foot building located in the Grand Central Park Development of Conroe, Texas. The SHSU Center for Health Professions has continued to expand and now includes a 950-car parking garage and the new College of Health Professions building. These additions support the continued growth of medical and health professions education on the Conroe campus.

The COM Building includes a Simulation Center, Standardized Patient Suite, a large Teaching Theater, an active learning classroom, small group rooms, an anatomy lab, research labs, an Osteopathic Clinical Skills Training Room, and dedicated student space where learning communities are housed.

Accreditation & Affiliation

COM Accreditation Status SHSU-COM is accredited by the Commission on Osteopathic College Accreditation (COCA) and by the Commission on Colleges of the Southern Association of Colleges and Schools.


Accreditation COCA guidelines are published on the AOA website. Following the comprehensive initial accreditation site visit and COCA review, SHSU-COM was granted full accreditation with exceptional outcomes in May 2024. The next scheduled site visit and comprehensive accreditation review is scheduled for 2034.

Curricular Offerings

SHSU-COM offers the Doctor of Osteopathic Medicine Degree. The curriculum is a horizontally and vertically integrated, systems-based curriculum with an emphasis in epidemiology and rural medicine. Clinical rotations will take place in one of seven major affiliated regions for SHSU-COM which include over 75 hospitals and/or healthcare facilities.

During the third and fourth years at SHSU-COM, students transition fully into clinical training through a distributive clinical education model that partners with hospitals and clinics across Texas rather than relying on a single teaching hospital. Using an assignment algorithm and lottery that considers students’ hometowns, learners are placed into regional rotations with meaningful opportunities to serve rural and underserved communities.

Students complete all core clinical rotations, including family medicine, internal medicine, pediatrics, obstetrics and gynecology, psychiatry, emergency medicine, and surgery, along with a dedicated month focused on rural and underserved care, with options for extended immersion for those drawn to these settings.

SHSU-COM places strong emphasis on clinical readiness, and preceptors consistently note that its students arrive exceptionally well prepared, contributing effectively to patient care from day one. This readiness reflects a rigorous pre-clinical foundation and is reinforced by strong board outcomes.

Clinical training is centered largely in East Texas, with ongoing expansion of rotation sites, and osteopathic principles remain fully integrated throughout, ensuring graduates gain broad, hands-on experience that prepares them for any residency path.

Accepted DO students may apply for consideration into a dual enrollment in SHSU's Master of Public Health program. Coursework for the MPH program is completed online in an asynchronous format. Curriculum is primarily taken during the summers prior and after OMS1 and completed during a fifth year after the conclusion of OMS4.
